About the Role One of my local government clients is seeking a Hospital Liaison Officer to join the Adult Social Care Reablement and Safeguarding team . This role supports the delivery of Section 2 and Section 5 notifications under the Care and Support (Discharge of Hospital Patients) Regulations 2014, ensuring effective communication between hospitals and social care services. The successful candidate will provide high-quality clerical, administrative and liaison support to ensure safe and timely hospital discharges and accurate record keeping. Key Responsibilities * Act as the primary point of contact for all hospital referrals and discharge notifications * Log and process Section 2 and Section 5 notifications promptly * Liaise daily with hospitals to confirm discharge dates and clarify referral information * Maintain accurate administrative systems, records, and indexes related to hospital discharge activity * Provide statistical information and support reporting requirements * Handle telephone and in-person enquiries from professionals, patients, and carers * Attend multidisciplinary meetings and case conferences, taking minutes when required * Support the maintenance of office systems, equipment and supplies * Contribute to continuous improvement within the team’s quality assurance framework * Work in line with Council policies including Equal Opportunities, Customer Care, Data Protection and Safeguarding Requirements * NVQ Level 2 or 3 * Minimum 3 years’ experience as an Administrator or Liaison Officer * Strong knowledge of hospital discharge processes and Adult Social Care * Experience working to tight deadlines in a busy environment * Proficient in IT systems including databases, spreadsheets and email * Strong communication and interpersonal skills * Ability to manage multiple tasks, prioritise effectively and maintain accuracy under pressure * Experience dealing with the public, health professionals and partner agencies Safeguarding All staff are expected to safeguard the wellbeing of children and vulnerable adults at all times.
