As a Community Nurse, you'll manage your own caseload and work with a high level of autonomy, while remaining connected to a collaborative multidisciplinary team. You'll be supported with ongoing development and the tools needed to provide outstanding care in the community. Key Responsibilities • Deliver high-quality nursing care to children within the community. • Develop and review personalised care plans in partnership with families. • Work collaboratively with healthcare professionals to ensure coordinated care. • Maintain accurate clinical documentation and manage your caseload effectively. About You • Registered Nurse with a valid NMC registration. • Competent in administering IV therapies. • Previous experience in a community nursing, home care nursing, district nursing, or similar role.
