This is a hands-on, fast-paced role where your assessment skills and ability to coordinate care will be key. 🔍 Key Responsibilities * Complete strengths-based assessments to support safe discharge planning * Work closely with patients, families, and carers to develop appropriate care plans * Assess capacity and risk, ensuring decisions align with legislation * Collaborate with multi-disciplinary teams (nurses, doctors, AHPs) * Arrange care packages, placements, and community support * Ensure smooth transitions from hospital to community services * Maintain accurate, timely case records ✅ What we’re looking for * Qualified Social Worker * Registered with the Scottish Social Services Council (SSSC) * Recent experience in adult social work (hospital discharge or community ideal) * Strong skills in assessment, care planning & risk management * Confident working in a fast-paced MDT environment * Excellent communication and organisational ability 📩 Interested?
