Adult Social Worker – Hospital Discharge Team Manchester City Council - Royal Infirmary Up to £30 per hour Manchester City Council is looking for a resilient and skilled Adult Social Worker to join the integrated team at the Royal Infirmary. This role is pivotal in ensuring safe, timely, and effective discharges from a busy acute hospital setting into the community. Key Responsibilities * Discharge Planning: Collaborate with the multidisciplinary Discharge Team to manage complex patient transitions. * Assessments: Complete Care Act 2014 Strengths-Based Assessments for Pathway 2 and Pathway 3 care home placements. * Safeguarding: Lead cases related to the Manchester Foundation Trust, ensuring Making Safeguarding Personal (MSP) principles are at the heart of every discussion. * Decision Making: Attend and contribute to Best Interest Meetings to facilitate safe discharge decisions. * Referrals: Manage specialized referrals including Neighbourhood Apartments and Crisis Clean services. Working Pattern Due to the focused nature of hospital work and limited space at the MRI office, we operate a strict hybrid model: * On-Site: Presence is required at the MRI during designated Duty Days. * Remote: Working from home is essential on non-duty days to manage office capacity. * Weekends: You will participate in a 1-in-7 weekend rota in conjunction with the Hospital Discharge to Assess team based in Gorton. Requirements * Registered Social Worker (Social Work England). * Proven experience in adult safeguarding and hospital discharge settings. * Strong knowledge of the Care Act 2014 and Mental Capacity Act. * Ability to work pacey, complex cases with a strengths-based approach.
