Role Overview

We are seeking an experienced Band 6 Community Occupational Therapist to join the Community Therapy Team in West Sussex, with a base at Horsham and cross-site working. You will provide specialist occupational therapy interventions to a varied community caseload, supporting individuals to maximise independence and quality of life.

Key Responsibilities

Undertake detailed assessments and develop tailored rehabilitation plans.

Provide clinical interventions to meet individual patient goals.

Collaborate with the MDT, patients, and families to ensure holistic care.

Maintain accurate documentation in line with HCPC and NHS standards.

Manage own caseload effectively and prioritise workload in a community setting.

Essential Requirements

HCPC registration as an Occupational Therapist.

Previous experience in community or rehabilitation settings.

Excellent clinical reasoning, communication, and organisational skills.

Ability to work independently and travel between sites.

Assignment Details:

Dates: ASAP ongoing

Hours: 08:00 - 17:00, Monday-Friday

Location: Horsham & cross-site within West Sussex
