Job Description - RGN Enfield Co Meath - Part-time 20 hours
Job Summary:
BSC in General Nursing or Registered Intellectual Disability Nursing with X experience
2-5 years of Community or general experience essential along with NMBI registration and full driving licence/private vehicle (travel essential)
Proficient in IT systems and Microsoft packages such as word and excel essential.
Desire to achieve/have already achieved post-registration qualification in healthcare management
Ability to balance case management responsibilities in conjunction with field work
Attention to detail, having a systematic and structured approach to tasks and processes essential.
Excellent written and oral communication skills to liaise with persons of all levels of nursing to coordinate patient services
Exhibits a passion for nursing, pursues continuous improvement, and advocates for clients' and nurses' viewpoints
Responsibilities
General
Be an active member of the Clinical Leadership Team in the provision of Adult Care in the community.
Provide leadership and clinical supervision to staff thus improving standards and championing a high morale amongst the team.
Lead a team of community-based staff in the provision of excellent evidenced based adult care
Develop and implement all aspects of the care plans based on the specific needs of the client and review accordingly.
Provide accurate reports and information to the relevant departments and Quality and Safety committee as requested
Anticipate and identify service improvements as part of our Continuous Quality Improvement program.
Work with the Operations team in ensuring that staffing is in place.
Carry out regular audits to ensure compliance with safeguarding systems in line with Policies and Procedures.
Liaise with multidisciplinary teams in the acute and community settings.
Identify staff training needs and provide accordingly.
Participate in the interview process by assessing candidates and ensuring high standards in recruitment selection.
Reporting to X
Care/Case Management
Compile and develop clinical care plans to meet the specific client needs
Complete timely and regular clinical reviews to ensure ongoing relevance of care plans
Meet with the Families/Clients/HSE to fully onboard new cases to ensure the specific needs to all are met and exceeded.
Carry out detailed assessment of new cases to ensure all aspects of the onboarding process is tracked and completed as required. Orientation and Induction of new staff to cases including providing mentorship and shadow shifts.
Establish and maintain strong relationships with all stakeholders essential.
Carry out detailed investigations into all Incidents/Complaints and ensure they are recorded and action to prevent recurrence.
Provide hands on clinical care in conjunction with clinical leadership
Compliance
Conduct visits to Client/Families/HSE to ensure that the needs to all stakeholders are met.
Carry out specific audits and clinical assessments as required to ensure compliance to all policies and procedures.
Complete weekly/monthly/quarterly reports as directed by clinical compliance and governance program ensuring quality care is provided whilst adhering to the company's policies and procedures
Ensure staff on relevant cases have up to date compliance with relevant mandatory training requirements
Work in conjunction with the Clinical Management Team to ensure all policies and procedures are up to date and in compliance
Staff
Provide Leadership and clinical supervision to Nursing/HCA staff members.
Meet with the Nursing/HCA staff to fully onboard new cases to ensure the specific needs to all are met and exceeded.
Ensure all Nursing/HCA staff work to a high standard of care at all time
Carry out induction and onboarding of all new staff members as well as ensuring all CPD and relevant training is up to date.
Provide direct Nursing supervision to ensure the strict adherence to company policies, Procedures, guidelines, and care plans and participate in service improvement and service safety initiatives
Work directly with HR as required on selection, recruitment, and management of relevant staff.
Rostering and Scheduling
Support the Client Services Coordinator rostering/scheduling to ensure the safe delivery of care and staff possess the appropriate skill set
Supervise and provided oversight on shift management to ensure the delivery of the service to clients
Have a degree of flexibility and availability to ensure shift fill goals are met participate in problem solving for any rostering issues with an awareness of nurse fill rates for clients.
