Community Nurse - Band 5  Whitton Start Date: ASAP Duration: Ongoing Pay: £22/£23 per hour Type: Full time or Part time (Min 3 days)  What: Community Nurse (Band 5) Main duties of the job Delivering comprehensive nursing care to patients in their homes, covering wound care, medication administration, and chronic disease management Assessing patient needs and crafting personalized care plans tailored to their unique circumstances. Empowering patients and their families with knowledge and strategies to manage their health effectively. Collaborating seamlessly with other healthcare professionals, including GPs, social workers, and occupational therapists Contributing to the development and implementation of innovative community nursing initiatives Keeping patient records organized and up to date to ensure seamless care coordination To excel in this role, you'll bring: Registered Nurse (RN) status with valid Nursing and Midwifery Council (NMC) registration Proven experience as a Community Nurse or in a relevant clinical setting Exceptional communication and interpersonal skills to build trust and rapport with patients. The versatility to work independently and collaboratively as part of a team. Impeccable organizational and time management skills to navigate a busy schedule.
