Qualified Social Worker - Merton - Adults Locality Team We currently have a short-term role for a locum Qualified Social Worker for a project to re-assess customers who have a double up care package. We are looking for a confident and skilled locum Qualified Social Worker to joint work with our Occupational Therapy Team in order to deliver this project. Our approach is strength-based, and we aim to achieve the best outcomes - enabling people to be safe and as independent as possible. This role of locum Qualified Social Worker will be working very closely with a qualified and experienced Occupational Therapist; but be supervising by the Team Manager or Assistant Team Manager from the Locality Team 1. You will work with service users, family carers and commissioners to ensure that support packages comprehensively meet assessed needs, whilst also being innovative and cost-effective. Our Teams Locality Teams 1 and 2 in Merton There are two Locality Teams (1 & 2) within Merton Adult Social Care, that are responsible for reviewing or re-assessing older people, people with physical and sensory disabilities. The two Locality Teams bring Social Workers together with NHS colleagues, supporting holistic, integrated working. Benefits in this role: * Supportive team and management * Flexible working available * Long term contract * Progression opportunities Your main responsibilities include: * Hold and effectively manage a complex and varied caseload with appropriate supervision, guidance, and support, reflecting the function of the team and the Vulnerable People Service in accordance with policy, procedures, guidance and legislation. * Undertake work within legislative frameworks including the preparation of written statements and representations to the judiciary at Court hearings. * You will be accountable for complex decision making in relation to caseloads ensuring professional judgement, service user involvement, needs led assessment, critical reflection, and analysis to inform this. * You will identify and assess levels of risk and need (within statutory frameworks) in often complex situations. Undertake safeguarding investigations and deliver protective and/or supportive services for individuals at risk and maintain up to date assessments, care plans and reviews. * You will be preparing statements of need/care and support plans, in consultation with other agencies as appropriate, that reflect the wishes of the individual and lead to the commissioning of personalised packages of care. * You will undertake direct work with adults and their families in line with care plans. * You will work co-operatively with both internal and external colleagues across multi - agency boundaries.
