Care Coordinator

Field Care supervisor Preston £22,800pa, plus on-call payment Responsibilities: * Support the delivery of high-quality care support services. * Assess and review individual services. * Provide practical supervision, instruction, and support to care workers. * Work with care coordinators to plan realistic care worker rotas. * Conduct regular reviews of individual services and needs assessments. * Participate in multi-disciplinary reviews of service users' needs. * Shadow new care workers to ensure their competence. * Conduct on-site observation and evaluation of care worker practice. * Spend the majority of working time in the community but also engage in office-based administrative work.
