Locum Band 6 Physiotherapist - In-patient Rehabilitation – Farnham, Surrey - £28p/h £28 per hour Mon-Fri, 08:30-16:30 3-month contract Farnham, Surrey, GU9 HCPC Registered with minimum 1 years’ experience We are currently seeking a skilled and compassionate Locum Band 6 Physiotherapist to join our in-patient rehabilitation team. The successful candidate will play a pivotal role in providing specialist physiotherapy services to patients with complex and/or chronic multi-pathology presentations. This locum position involves working within a multidisciplinary team to deliver comprehensive rehabilitation interventions aimed at optimising functional independence and quality of life for in-patients. Clinical Assessment and Evaluation: * Conduct thorough assessments of patients admitted to the rehabilitation unit, including comprehensive musculoskeletal, neurological, and functional assessments. * Utilise standardised outcome measures and clinical observations to establish baseline function, identify impairments, and formulate individualised treatment plans. Treatment Planning and Intervention: * Develop evidence-based treatment plans tailored to the specific needs and goals of patients with complex and/or chronic multi-pathology presentations. * Provide specialised physiotherapy interventions, including therapeutic exercises, manual therapy techniques, gait training, balance retraining, and functional mobility training. * Progressive Rehabilitation Programs: * Implement progressive and goal-oriented rehabilitation programs to address physical impairments, functional limitations, and activity restrictions. * Collaborate closely with patients, their families, and the multidisciplinary team to establish realistic rehabilitation goals and priorities. Chronic Disease Management: * Take a holistic approach to managing chronic health conditions and multi-pathology presentations, including cardiovascular diseases, respiratory disorders, musculoskeletal conditions, and neurological disorders. * Provide education, self-management strategies, and lifestyle interventions to optimise long-term health outcomes and quality of life. Pain Management and Symptom Control: * Assess and manage pain, discomfort, and other symptoms experienced by patients with complex and chronic conditions, utilising a combination of therapeutic modalities and pain management techniques. * Monitor patient responses to interventions and adjust treatment plans accordingly to achieve optimal pain relief and symptom control. Patient and Family Education: * Educate patients and their families about their condition, rehabilitation goals, and strategies for maximising functional recovery and independence. * Provide training on safe mobility techniques, assistive devices, and home exercise programs to support continued progress after discharge. Discharge Planning and Transitional Care: * Collaborate with the multidisciplinary team to facilitate safe and timely discharge planning, ensuring continuity of care and appropriate community support services. * Liaise with community rehabilitation teams, social services, and other stakeholders to coordinate post-discharge care and support. Clinical Documentation and Record Keeping: * Maintain accurate and up-to-date electronic health records, documenting assessments, interventions, progress notes, and discharge summaries in accordance with professional and regulatory standards. * Ensure clear and concise communication of patient progress and treatment plans within the multidisciplinary team. * Quality Improvement and Professional Development: * Participate in clinical audits, quality improvement initiatives, and research projects to enhance the quality and effectiveness of rehabilitation services. * Engage in ongoing professional development activities to stay abreast of current evidence, best practices, and advancements in physiotherapy practice. Qualifications and Requirements: * Bachelor's or Master's degree in Physiotherapy. * Current registration with the HCPC. * Minimum 1 years’ UK experience * Significant experience working as a Physiotherapist in in-patient rehabilitation settings, preferably with patients with complex and chronic multi-pathology presentations. * Advanced clinical skills in musculoskeletal and neurological assessment, treatment planning, and rehabilitation interventions. * Strong communication, interpersonal, and teamwork skills. * Ability to work autonomously and collaboratively within a multidisciplinary team. * Commitment to delivering patient-centered, evidence-based care with compassion and empathy
