Assertive Outreach Worker - Liaison and Diversion


Our client is a non-profit organisation working with people to support their health and wellbeing. Established for over 30 years, they have provided services alongside a variety of statutory and voluntary, community and social enterprise (VCSE) colleagues in Surrey and its borders. They believe change is achievable when offered by a flexible, respectful, inclusive and a non-judgemental approach.

They work in Partnership with Surrey Borders Partnership NHS Foundation Trust and are currently looking for an enthusiastic, caring, organised Assertive Outreach Worker to work within the Surrey Liaison and Diversion Service. The role provides Outreach Support in the Surrey Community.

Salary: up to GBP27,000 Per Annum dependent on experience

Hours: 35 hours (Monday to Friday)

Duration: Permanent

Location The service covers clients within the Surrey area, with the main office currently based in Guildford (this may be relocated to Frimley in the future). The role involves blended working, some travel to visit clients in the Surrey area (example Woking, Guildford, Staines, Chobham, Runnymede, Redhill etc). Travel expenses including mileage are reimbursed. Would suit car owner driver.

The Assertive Outreach Worker will work alongside colleagues in the Liaison and Diversion Service to offer person-centred and community based support and interventions, aiming to improve the health and social care outcomes for individuals with the ultimate goal of reducing offending.

The Liaison and Diversion Service is passionate about improving the lives of individuals by providing supportive early intervention for vulnerable people from the age of 10 who come to the attention of the criminal justice system.

They provide assessments to identify vulnerabilities such as: mental health concerns, physical health concerns, learning disabilities, substance misuse concerns, housing, finance concerns, and other vulnerabilities. The Assertive Outreach Team are then able to offer and provide support for the identified vulnerabilities which can involve signposting, referrals to appropriate support services, assistance with making and attending appointments based on their needs, collaborative working with other services and organisations, the Assertive Outreach Team also provides well being support in a person centred holistic way.

This post is subject to an enhanced DBS check. Please note past drug and/or alcohol or criminality history will not necessarily discount you from undertaking this role

Our ideal applicant will have/be;
  • Enthusiastic, with a drive to provide excellent person centred care, support and planning
  • Willing to learn, and seek to improve quality of care
  • Previous community experience working with individuals relating to mental health, learning difficulties, drug and alcohol addition, homelessness etc
  • Knowledge of the criminal justice system and / or Health and Social Care is desirable
  • Ability to work collaboratively within a multidisciplinary team
  • Working knowledge of assertive outreach work, assessing and managing risk and safeguarding, policies and procedures supporting these
  • Good communication skills both written and verbal
  • Proficient in Microsoft Word, Outlook and Excel, with previous experience of an electronic patient record system preferrable
  • Ability and willingness to travel and work flexibly
