Team Manager | Leaving Care | London Borough of Lambeth* Team Manager required to lead and manage a statutory Leaving Care Service Hybrid working Office base: London Borough of Waltham Forest* You must have a social work qualification and full SWE registration as well as relevant experience to be considered London Pledge Rates Apply - £44.50Manage a team of up to 6 SW. who will need to complete Needs Assessments and Pathway Plans for Children and Young People in the Leaving Care Service. Meet all the key performance indicators such as visits, pathway plan, CLA reviews, Case Summaries, Chronologies, SDQs, Review Health and Dental Assessment for children looked after. This is an great opportunity to work with an established team and with our community to provide a service to children in care. We want to offer an outstanding level of support to our children in care and leaving care. Responsibilities: Provide the 121 supervision of staff, Be responsible for a manageable caseload, working with a dedicated team to improve the lives of the children requiring specialist services. Managing the KPIs for children looked after and providing a high quality service. Ensuring that children's care and development needs are met within placements by undertaking necessary placement management, planning and intervention. To be considered, it is essential that you have a recognised Social Work qualification (Social Work Degree or Diploma in Social Work) and full Social Work England registration as well as recent or significant relevant experience in a similar role.
