This role is situated within our Leaving Care Service, where the primary responsibility is managing a caseload of young people preparing to leave care. The position involves collaboration with allocated Pathway Advisors who are also part of our service team. Early engagement with young people transitioning from other services is a key aspect, and you will take the lead on completing pathway plan assessments and the initial pathway plan. Maintaining these plans in compliance with the required legislation until the young person reaches 18 years old is a crucial responsibility, at which point they fully transition to their allocated Pathway Advisor. Additionally, you will participate in our duty system, providing extra support to our young people alongside our Pathway Advisors.
Responsibilities
Manage a caseload of young people preparing to leave care.
Collaborate with Pathway Advisors to support young people.
Lead the completion of pathway plan assessments and initial pathway plans.
Maintain pathway plans in accordance with legislative requirements until the young person turns 18.
Participate in the duty system to provide additional support.
Person Specification
Proven experience in social work, particularly with young people transitioning out of care.
Strong understanding of legislation related to care leavers.
Excellent communication and collaboration skills.
Ability to manage a diverse caseload effectively.
Commitment to supporting young people in achieving positive futures.
