About the Role:

As a Care Coordinator, you will be responsible for coordinating and managing the delivery of care services to clients in the Gloucester area. You will work closely with the care team to ensure the highest standards of care are maintained at all times.

Requirements:

• Minimum of 2 years of experience in a care coordination role
• Knowledge of domiciliary care regulations and best practices
• Excellent communication and interpersonal skills
• Strong organizational and time-management abilities
• Ability to work independently and as part of a team
• Proficient in IT and administrative tasks
• NVQ Level 3 in Health and Social Care (or equivalent)

Responsibilities:

• Coordinate and manage the delivery of care services to clients
• Conduct initial assessments and create care plans for new clients
• Schedule and allocate care workers to clients based on their needs and preferences
• Monitor and review care plans to ensure they are up to date and meet the changing needs of clients
• Maintain accurate and up-to-date records of care services provided
• Liaise with clients, their families, and healthcare professionals to ensure the best possible care is provided
• Manage and resolve any issues or concerns raised by clients or care workers
• Provide support and guidance to care workers, ensuring they have the necessary training and resources to provide high-quality care
• Participate in on-call duties on a rotational basis
• Keep up-to-date with changes in care regulations and implement any necessary changes to policies and procedures.
